How to Order Prescription Medications
Thank you for your interest in the Revival Animal
Health Pet Pharmacy! As we continue to grow, we look forward to providing you additional
products and services to assist you with all your prescription needs.
You can
place a prescription order online or by calling one of our Personal Pet Care Pros at
800-786-4751. In order to receive prescription medications from Revival, we will need
the prescription from your veterinarian. We can accept prescriptions through these
methods:

Download our Prescription Authorization Form or you can also
call us at 800-786-4751 or email us at rx@revivalanimal.com to receive
the form. Your veterinarian will need to fill out the form and fax it back to us at
877-335-2680.
Fax
Your veterinarian can fax the original
prescription from their office to our pharmacy at 877-335-2680. We can only accept
faxed prescriptions from a veterinarian, so please do not fax the prescription
yourself.
Phone
Your veterinarian can call our pharmacy directly at
712-737-5555 x134 and authorize the prescription. If our Pharmacists are not
available, please leave a message. All prescription authorizations will require an
order and payment information before we can ship the medications. If your
veterinarian has sent in a prescription form, please place an order online or over
the phone.

All prescriptions will remain on file at Revival. Prescriptions are valid for one year
or until the total quantity has been dispensed, whichever happens first.

Pharmacy Laws
A prescription must contain the following
information prior to being filled at a pharmacy.
Name of owner
Name and species of the animal
Name of drug
Strength of the drug
Dosage form of the drug
Quantity of the drug
Directions of use
Date of issuance
Vet's name with signature and state license number
Number of refills authorized
A prescription may be transmitted by facsimile (fax) to the
pharmacy only by the prescribing veterinarian. The prescription can not be
faxed to the pharmacy by the owner of the animal. The owner may obtain the original
prescription from the vet and mail it in.
Federal (U.S.A.) law restricts the
use of Rx drugs to be used by, or on the order of, a licensed Veterinarian.
Please Note: Due to federal and state law, we cannot accept returns of
prescription medications, so please make your selections carefully.
Sorry, needles and syringes cannot be shipped to Delaware, New York, and Miami-Dade County, FL, without a prescription.
We cannot ship needles and syringes to the US Virgin Islands.
A limit of 10 needles or syringes may be sent to Connecticut, Minnesota & New Jersey.
A limit of 100 needles or syringes may be sent to Illinois.
A prescription is not required in Delaware, Illinois, Minnesota or New York if for livestock use. We will require a form to be completed prior to shipping your order.

Solo-Jec® 9 single dose vaccine for dogs gives broad protection against nine diseases in a convenient 1 ml single dose package with syringe. This 9 way puppy shot is recommended for the vaccination of healthy dogs six weeks of age or older and protects against diseases caused by Canine Distemper, infectious Canine Hepatitis (CAV-1), Canine Adenovirus Type 2 (CAV-2), Canine Parainfluenza and Canine Parvovirus (CPV), as well as four types of Leptospira (Leptospira Canicola, Icterohaemorrhagiae, Grippotyphosa, and Pomona).
The Solo-Jec® 9 single-dose vaccine for dogs contains the CPV 2b strain. The CAV-2 fraction cross-protects against respiratory infection caused by infectious canine hepatitis (CAV-1).The Solo-Jec 9 vaccine for dogs uses a modified live virus containing antigens that can replicate in the dog's systems to build immunity. The diluent contains killed antigens for Leptospira Canicola, Leptospira Icterohaemorrhagiae, Leptospira Grippotyphosa, and Leptospira Pomona bacterial extract.
The 1 ml single dose package includes a disposable syringe and needle.

The 9-in-1 Shot for Puppies and Dogs
The 9 and 1 puppy shot is a convenient way to prevent dangerous canine diseases like:
Distemper: This highly contagious, severe disease affects the puppy's respiratory, gastrointestinal and nervous systems.
Hepatitis: Hepatitis is an acute virus that spreads quickly and attacks dogs' spleens, kidneys, lungs and other vital organs.
Adenovirus type 2: This disease is spread through coughing and causes localized respiratory distress.
Parainfluenza: This highly transmissible infection affects dogs' respiratory systems.
Parvovirus: Commonly referred to as parvo, this contagious virus causes acute gastrointestinal distress in puppies.
Leptospira: This severe bacteria-borne disease mainly attacks dogs' lungs and kidneys.
For subcutaneous injection in dogs six weeks of age or older. Shake well before use.
Package Includes 1 (one) dose vial of dry vaccine 1 (one) ml vial of vaccine diluent 1 (one) disposable syringe and needle
Warning Only vaccinate healthy animals. Animals incubating any disease or stressed due to shipping, malnutrition, or parasitism may not achieve or maintain an adequate immune response.
Storage: Store out of direct sunlight at 35°-45° F (2-7° C). Do not freeze. Use entire contents when first opened.
Disposal: Burn vaccine containers and all unused contents.
Directions for Use: Open syringe by twisting or tapping the cap against a hard surface to break the heat weld. Prepare the vaccine by injecting the diluent into the vial containing the desiccated vaccine cake. Shake well. Withdraw entire contents into the syringe. Thoroughly prepare injection site with antiseptic. Lift the loose skin behind the neck or behind front leg and insert needle. Inject entire contents of syringe. Do not inject directly into blood vessel.
Inject subcutaneously 1 ml into healthy dogs six weeks of age or older. Revaccinate puppies at two to three week intervals until at least 16 weeks of age. All dogs over 12 weeks of age should initially receive one dose of Solo-Jec® 9 and a second dose two to three weeks later. Annual revaccination with a single dose is recommended.
