Chlorhexidine Gluconate 2%, Ketoconazole 1%.

Shake well. Wet hair with warm water. Apply sufficient amount to create rich lather. Massage into wet hair. Rinse and repeat. Allow to remain on for 5-10 minutes. Rinse thoroughly with clean water. May be used 2-3 times a week for 4 weeks then once a week.
